Eligibility for Employees

For most employees who quality, coverage begins the first month following your hire date. In order to be eligible for benefits you must meet the following criteria:

  • You must be an active full-time employee with at least 60 hours per pay period and be on the regular payroll to receive full-time premiums
  • You must be an active part-time employee with at least 48 hours per pay period to receive part-time medical premiums
  • An active employee, budgeted to work at least 40 to 47 hours per pay period prior to January 1, 2014
  • Those employees who have averaged 30 hours per week prior to January 1, 2014 may quality for the Look-Back Measurement Method noted in section C of your Tuality Healthcare Plan booklet

Eligibility for Family Members

A covered employee's family members are:

  • Lawful spouse
  • Domestic partner
  • Children from birth to age 26, or children who are dependent on you due to physical or mental disability, regardless of age, providing the disability existed prior to the age of 23

The term child or children shall include:

  • Natural children of the employee
  • Adopted child or children placed with a covered employee in anticipation of adoption
  • Foster children or children placed with a covered employee in anticipation of adoption
  • Stepchildren
  • Children of domestic partners
  • Legal guardians
